I started Form Floral in my second floor walk-up apartment 11 years ago. Today, we've got a dream team of talented and seasoned designers, and a list of clients that'll make you weak in the knees.

Owner & Creative Director
Born & raised in Phoenix, Mal has had every creative hobby in the books throughout her life. She found her home and passion in floral design and these days is mainly behind the scenes keeping the Form crew happy and the business running smoothly. You can also catch her over on Tiktok sharing design and business advice from her experiences.
General Manager
Tami has also worked in nearly every facet of the floral industry from wholesale to retail to event work including running her own business. She was with us at the shop and now she manages all of our event logistics, our team of designers, and our resort partnerships.
Operations Director
Kristina is our traffic director, manager of logistics, first one to welcome new clients into the Form fam, and the glue that holds us all together.
Event Manager & Designer
Coming to Form from an event planning background, Lauren is both a creative force AND an expert timeline-keeper/client-communicator. Find her on-site hauling in big ol' plants or behind her computer eating a homemade lunch that probably came from her garden.
Studio Assitant & Retail Designer
Nola is quick on her feet, always brings joy into every room, and comes to us from a long event industry career. Shifting from catering to floral was a no-brainer for her and we're honored to have her on the crew. If Nola isn't pulling inventory, cleaning our buckets, and wiring stems with expert precision, she's designing delivery orders for our retail clients; a Jill of all trades.
